- By filling in the form bellow and printing it by pressing tabs „Ctrl+p“on the keyboard (or with the right click of the mouse by pressing it on any place of the form), then select menu item „Print“) and send the filled-in form to us by fax: +370 41 520911.
- By filling in the form bellow and sending it to us by e-mail by pressing the button „Confirm“ below.
